What can Providers do - overview

Through out New Zealand, health providers are leading us toward better prevention and care for long-term conditions. Success is happening at every level of the health and disability sector. Individual successes may often be partial but together they make up a growing movement.

On this website we want to highlight success. We are focusing on nurse-led long term condition initiatives, because this work is central to the prevention and management of chronic conditions within our communities.
